Why “Normal” Bloodwork Doesn’t Always Tell the Whole Story
Most routine checkups screen for anemia by looking at hemoglobin, the protein in red blood cells that carries oxygen. If hemoglobin falls within range, many people are told their iron is fine. But hemoglobin is typically one of the last markers to drop when the body’s iron stores are being depleted. Ferritin, the protein that reflects how much iron is actually stored in the body, tends to fall much earlier—often years before anemia develops.
This gap matters. A growing body of research now distinguishes between iron-deficiency anemia and a separate, earlier-stage condition sometimes called iron deficiency without anemia. People in this second group can have completely normal hemoglobin and still experience real, measurable symptoms tied to low iron stores. It’s a distinction that’s easy to miss in a routine visit, but an important one for anyone whose fatigue has never quite added up.
The Symptoms That Often Get Blamed on Something Else
Low ferritin doesn’t always announce itself clearly. Testing: What to Ask For
A standard complete blood count alone won’t catch iron deficiency without anemia. A more complete picture usually includes serum ferritin alongside transferrin saturation and, when relevant, inflammation markers, since ferritin can be falsely elevated during inflammation. Many labs list “normal” ferritin as anything above roughly 15 ng/mL, but a growing number of clinicians use a higher threshold—often closer to 30 to 50 ng/mL—before ruling out iron deficiency as a contributor to fatigue-type symptoms, particularly in menstruating women. This is a conversation worth having directly with a healthcare provider, since interpreting ferritin in context takes some nuance.
